TOBACCO SURCHARGE FOR TI MEDICAL INSURANCE: If you and/or your covered spouse or domestic partner uses tobacco products, there will be an additional charge of $30 per month for each covered adult tobacco user, with a maximum of $60 per month. (The additional cost does not apply to residents of Kentucky.) You can avoid paying the tobacco surcharge by participating in a formal tobacco cessation program, as long as you are willing to attest that you are participating and regardless of whether you actually stop using tobacco products. Please refer to cessation program resources below.

National Cancer Institute: 1-877-44U-QUIT (1-877-448-7848) provides trained counselors to guide you through tobacco cessation. Help is available in English or Spanish, Monday through Friday, 8 a.m. to 8 p.m. Eastern time.

Your state quit line: 1-800-QUIT-NOW (1-800-784-8669) connects directly to your state’s quit line. All states have quit lines in place with trained coaches who provide information and help with quitting. Specific services and hours of operation vary from state to state.
